Spring Asymptomatic COVID-19 Testing Protocol Requirement

The University is continuing with the on-campus asymptomatic COVID-19 testing protocol for the spring semester, which is required for graduate students who meet any of the testing criteria listed below. Additionally, if you do not meet the criteria, but would like to opt-in to the testing protocol, you may do so beginning February 1, 2021. If you choose to opt-in, you will be required to participate according to the protocol (twice weekly) and will be held to the same compliance standards as those who are required to participate.

To that end, please review the testing criteria below and let us know if you meet the criteria for required testing, do not meet the criteria for required testing but wish to opt-in, or do not meet the criteria for required testing and do not wish to opt-in.

All graduate students must complete this form, even if you were enrolled in the testing protocol this fall.

Testing Criteria:

  • You are physically on campus this spring for at least eight hours or more on a weekly basis or;
  • Live in university housing or;
  • Attend an in-person class of any duration
